What is the format for extraneous material in a Table of Contents for a Grade 1 book?

There are no page numbers referencing the material.

Below is an example:

Look for Assessment Opportunities
(box line)

Student Book A and Student Book B

(bullet) Quick Check at the beginning of every …
(bullet) Guided Practice after every example or two …

Workbook A and Workbook B

(bullet) Chapter Review/Test in every chapter to …

Every other page in the contents has this type of text.

In order to assess how to handle the extranous material I need to see what it looks like. I need to see the entire print page so I can see how the different types of material printed on it relate to each other, especially if it’s a table of contents and some of the material is not part of the table of contents.

Please scan and post a print page or two.
